Saint Mary's Independent Living Extension (SMILE)
St. Mary’s Independent
Living Extensions (SMILE) is a non-profit agency in Lilburn providing supported living services to Developmentally Disabled
adults in their community. We work with individuals, their family members, and other providers to ensure
all of the individual’s needs are met. Finding the right services and supports for a loved one can be a daunting task. Although
the NOW and COMP Medicaid Waivers offer a number of different services, navigating the requirements and discovering how to
access these services can be very frustrating. At the end of the day, most family members simply want a
safe and comfortable environment for their “loved one.” SMILE was created with a vision and
mission to provide services to individuals with developmental disabilities in the community while maintaining a family atmosphere.

If you have already
researched the NOW and COMP Waivered Services, you will recognize that our Residential and Day Program services offer the
bulk of Independent Living Supports for Developmentally Disabled adults. We then coordinate with other
services providers to complete the full needs of people we serve. Our
current services provided directly by SMILE: Community Living Support services are designed to provide supports to participants who live in their own or family home.
Community Living Support services are individually tailored to assist with the acquisition,
retention, or
improvement in skills related to a participant’s continued residence in his or her own or family home. Community Access Services are designed
to assist the participant in acquiring, retaining, or improving self help,socialization, and
adaptive skills required
for active community participation and independent functioning outside the participant’s
home or family home.
Other Services, such as Supported Employment and Community
Residential Alternative, are Coordinated with Other Providers.

Work in partnership with the individual, their families, and other agencies to maximize
each person’s potential · Identify and remove barriers that make participation in the community difficult ·
Assist and support the individual’s independent personal care and living ·
Support for creative and unique solutions to overcome barriers to successful living ·
Assist the individuals to be active in their communities and find ways for
everyone to give back to the community
